Alberta Seniors and Community Supports developed the Personal Directives Registry to enable Albertans to voluntarily register information about their personal directives. The Registry provides a mechanism for health care providers (Approved Service ProvidersA Regional Health Authority or Physician who has signed an agreement with the Office of the Public Guardian specifying the terms and conditions under which they, or their authorized employees, agents, contractors or other Physicians who practice with the Approved Service Provider may access the Registry. – e.g. Physicians, Regional Health Authorities) and Authorized UsersAn employee, agent or contractor of an Approved Service Provider or a Physician who is practicing with the Approved Service Provider, when the Approved Service Provider is a physician, who is authorised to perform searches of Registry information. (employees, agents,A person who is designated in a directive to make decisions on behalf of the maker. contractors of Approved Service Providers) to find out if a person under their care has a directive and how to contact their agent(s), if any. The Registry does not contain copies of personal directives.

If you are a MakerA person who has completed a personal directive. of a personal directive who voluntarily supplies your personal information by registering in a secure portion of this site, your information is collected in compliance with the Personal Directives Regulation under the Personal Directives Act and section 33 of the Freedom of Information and Protection of Privacy Act. We are collecting your personal information so that Approved Service Providers and their Authorized Users can find out whether you have a personal directive, in the event that you lose your capacity to make personal decisions, and who you have named as (an) agent(s) for personal decision-making. Approved Service Providers and Authorized Users access the Registry on a need to know basis.

If you are an Approved Service Provider, Authorized User or Contract AdministratorAn employee of an Approved Service Provider delegated by the Approved Service Provider to designate Authorized Users, within the place of employment, to maintain a current list of Authorized Users and to notify the Office of the Public Guardian of any changes to the list. your personal information is collected in Compliance with the Personal Directives Regulation under the Personal Directives Act and section 33 of the Freedom of Information and Protection of Privacy Act. It is collected in order to administer and maintain the Registry program and the Registry web site and to keep the list of Authorized Users for Approved Service Providers accurate and current.

You have a right to access information about you that is in the Registry.

Your personal information is used, disclosed, protected, retained and disposed of in compliance with Part 2 of the Freedom of Information and Protection of Privacy Act, the Health Information Act (where applicable), the Personal Information Protection Act (where applicable) and in compliance with the Personal Directives Act and Personal Directives Regulation.

If you are an Approved Service Provider or Authorized User who is searching for information on the Registry, we will be tracking the records that you accessed against the Registry user ID.

All Government of Alberta web sites, including ours, collect usage statistics including the Internet Protocol (IP) address of the computer that contacts the web site but we will not be doing anything specific to track this information. We will only use “cookies” for the following purposes: to store the size of font (for text size) that you want to use (expires after a day) and to track information specific to the session (expires when you close your browser). However, in order for Makers to register their personal directive information and for Approved Service Providers and Authorized Users to access the Registry, you and they will need to obtain a password through the Alberta Secure Access System (ASAS). That system does use cookies to confirm login and identity.

For site security purposes and to ensure that this service remains available to all users, we use software programs to monitor network traffic to identify unauthorized attempts to upload or change information, or other wise cause damage. Also, in order to monitor usage of the Registry, we will track change date and “change by” information.

The Registry web site may be linked to other Government of Alberta web sites, including the Office of the Public Guardian section of the Alberta Seniors and Community supports public web site. It may also be linked to the Service Alberta ASAS site. We are not responsible for the content and the privacy practices of other web sites and encourage you to examine each site’s privacy policy and disclaimers and make your own decisions regarding the accuracy, reliability and correctness of material and information found.
